Summary

The facility failed to ensure medications were available as ordered for a resident admitted after a right foot amputation and with diagnoses including anxiety disorder, chronic kidney disease, depression, and diabetes mellitus with skin complications. The resident was cognitively intact with a BIMS score of 15 and had physician orders for oxycodone 5 mg, 2 tablets every 4 hours as needed for pain, and lorazepam 1 mg every 8 hours as needed for anxiety, both starting on the admission date. The resident stated that she went without oxycodone for about 3 to 4 days and was repeatedly told by nursing staff that the pharmacy had not delivered it. She also stated that she went without lorazepam for about 3 to 4 days and was told the pharmacy had not delivered that medication either. Nursing and administrative staff confirmed ongoing problems with the new out-of-state pharmacy, including delayed deliveries, missing stock, and confusion about whether the oxycodone prescription was valid. Staff interviews and record review showed that the oxycodone was not available in the facility when requested, emergency kit supply was unavailable or had an incorrect count, and the medication was not received until after delays. The MAR showed the resident received oxycodone on the admission date and then not again until two days later, and the MAR did not document lorazepam doses on two consecutive days near the end of the month. The resident was also sent to the local hospital by ambulance for anxiety symptoms and chest pain after reporting she had not had lorazepam for several days, and hospital documentation recorded that she had not received her anxiety medication because the SNF had run out.
