Posted Menu Not Followed for Dessert Items

El Paso Rehabilitation And Health Care CenterEl Paso, Illinois Survey Completed on 06-10-2026

Summary

The facility failed to provide the posted diet for three residents in a sample of 36. The facility’s Spring/Summer Master Menu 2026, week 3, listed a noon meal on Monday, June 8 that included a homemade frosted banana cupcake, and a noon meal on Tuesday, June 9 that included yellow cake with chocolate icing. However, during the Resident Council Meeting on 6/08/2026, three residents voiced concerns that they only received plain cake with no frosting or icing. During the noon meal observation on 6/08/2026, the three residents were served turkey cutlet, mashed potatoes and gravy, vegetables, and a banana cupcake with no frosting. The Dietary Manager confirmed the cupcake had no frosting and stated she would ask the cook why the cupcakes were plain. During the noon meal observation on 6/09/2026, the same residents were served a chicken taco, beans, coleslaw, and a piece of plain yellow cake with no frosting or icing. The Dietary Manager confirmed the yellow cake did not have chocolate frosting and stated she would talk with kitchen staff. The Administrator later stated kitchen staff should always follow the established facility menu, including frosting or icing all cakes, cupcakes, and brownies as ordered.
