Failure to Provide Ordered Wound Care and Assess New Wounds
Summary
The facility failed to ensure prescribed wound treatment was provided for a resident with a venous ulcer of the left lower leg. The resident had diagnoses including type 2 diabetes, peripheral vascular disease, and an acquired absence of the right leg above the knee. During observation, the resident’s left lower leg was reddened, dry, scaly, and had multiple white patches of cream scattered on the leg with open areas noted. The RN cleansed only the distal portion of the leg with wound cleanser, did not wash off the white cream, and then applied A & D ointment, medicated cream with moist gauze, and an elastic bandage. The RN identified the white cream as zinc oxide from a previous treatment order, and the wound nurse stated the resident’s treatment orders had recently changed and that staff should have been using soap and water to remove the cream before cleansing with wound cleanser. Record review showed the resident’s wound treatment orders had changed over time, with one order discontinued and another active order for cleansing with Hibiclens, applying A & D ointment to the peri-wound skin and left lower extremity, and applying Dakin’s half-strength moistened gauze, calcium alginate, absorbent pad, kerlix, and elastic bandage twice daily. The treatment administration record showed both the discontinued and current treatment orders were signed off as administered from 7/1/26 through 7/6/26. The wound nurse also stated she did not see an order for zinc cream to be applied, although she knew it had been used previously. The facility also failed to assess open wounds and obtain treatment orders for another resident. That resident had diagnoses including diabetes, muscle wasting and atrophy, and schizoaffective disorder. The resident was observed with bandages on the right knee, scabs on both feet and lower legs, and later was found to have two open wounds on the right knee after the bandages were removed. Nursing staff stated they had not seen new wounds or bandages on the knee during prior shifts, and one nurse stated that when there are new wounds, they assess the wounds, notify the wound nurse, and notify the physician for treatment orders. The wound nurse stated she was first made aware of the wounds when she assessed the resident and that dressings should not be applied without an order. The physician order summary did not show an order for the right knee until after the wound evaluation was completed.
