Food Served at Unpalatable Temperatures and Lacking Seasoning

Aperion Care BurbankBurbank, Illinois Survey Completed on 07-09-2026

Summary

The facility failed to ensure food was served at temperatures that were palatable and attractive, affecting all 48 residents who consume food and beverages from the kitchen. The census showed 50 residents in the facility, with 2 residents NPO. The grievance log from 1/1/2026 through 7/6/2026 documented nine concerns related to the facility's food and food preferences. Two residents interviewed described the food as cold, unseasoned, and generally poor in quality; one cognitively intact resident stated the food was cold and tasted bad, while another resident with cognitive impairment said the food often came to the room cold and did not have any seasoning. During observation of the lunch meal, a cook prepared a test tray from the steam table that included ranch baked chicken, potato wedges, and buttered carrots, but the margarine, cake, and beverage were not provided with the tray. The potatoes appeared to be chunks of cut-up potatoes with skin on and lacked seasoning. Although the dietary manager confirmed the thermometer was calibrated and the steam table was operational, the steam table produced minimal steam and felt only mildly warm when checked by hand. Temperatures taken on the test tray were 123 F for chicken, 91 F for potatoes, and 102 F for carrots initially, and at point of service were 105 F for chicken, 91 F for carrots, and 88 F for potatoes. The dietary manager affirmed food should be served at a palatable temperature, and the DON and NP acknowledged resident complaints about unseasoned food and food not matching residents' cultural preferences.

F0804 F804: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.
Short Summary

Two residents were served microwave-reheated items without a temperature check before service. A staff member reheated almond milk and chicken noodle soup, placed both on the counter, and they were later measured at 182 F and 190.4 F, respectively. Staff stated the kitchen checked temperatures, but the items were served before confirming they had cooled to a safe temperature.

Short Summary

Food portions were not served per recipe directions when a cook prepared sandwiches, pureed items, cake, and side items by estimation instead of measured amounts. The cook used tongs to add unmeasured meat, onions, and cheese to buns, served visibly different cake portions, and confirmed there were not enough pickles for all residents. The cook stated the portions were “eye balled” and that the spoons were inconsistent.
