Dish Machine Not Sanitizing Cups and Utensils

Austin Oasis, TheChicago, Illinois Survey Completed on 07-31-2026

Summary

The facility failed to sanitize kitchen equipment in accordance with its low-temperature dishwashing policy. On 07/28/26, a resident brought a cup of juice to the surveyor and stated there was a roach in it. The surveyor observed a tiny oval-bodied insect floating on top of the liquid in the resident’s plastic cup. A PM cook identified it as a roach, and the dietary supervisor stated the cups and juice dispensers are washed after every meal using the dishwashing machine, but the insect should not have been there. During observation of the dishwashing process, a dietary aide was seen feeding dirty plastic cups into the dish machine and stated that cups are sent through again if they do not look clean. When the dish machine was tested, the chlorine test paper remained white, and the dietary supervisor stated there was no chemical sanitizer in the barrel because the sanitizer level was too low to feed into the machine. The supervisor stated she did not know how long the machine had been without sanitizer and that staff are supposed to check it before every meal. The supervisor later stated the items being put into the dish machine were not getting sanitized and that this was a problem for resident safety. The facility’s records showed 173 residents in the building, with one resident NPO. On 07/29/26, a technician arrived to fix the dishwasher after it was observed not cleaning dishes properly. Additional interviews with residents and staff described repeated concerns that plastic cups were cloudy, had residue, or were dirty, and one CNA reported seeing a live roach in a cup the prior week. The registered dietitian stated the low-temperature dish machine uses chemical sanitizer to kill bacteria and prevent cross contamination and infections, and that without sanitizer nothing being washed in the dishwasher is getting cleaned.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
