Inadequate CNA Staffing on Dayshift

Summary

The facility failed to ensure that direct resident care staffing was adequate to meet resident needs. On 6/28/2026, the day shift began with only 2 CNAs scheduled instead of the usual 3. One CNA became sick and vomited at the nurse’s station and was sent home, leaving 1 CNA on the floor after the night-shift CNA who had stayed over left at 10:00 AM because he had already worked 12 hours. Staff stated that from 10:00 AM until shortly before 2:00 PM, only 1 CNA was available on the floor, while nurses tried to help with feeding, passing trays, and some showers in between their other duties. The DON was notified of the staffing shortage but did not come in to assist, and the ADON/MDS Coordinator stated she was unable to come in because of family plans. Residents described delays and missed care during the staffing shortage. One resident stated call lights were going off for a long time, residents needing assistance had to wait, and she had an episode of incontinence because staff could not get to her in time. She also stated her showers were scheduled twice weekly but she did not receive showers on two scheduled days the prior week because staff did not have time. Another resident stated she was not happy with her care, did not get showers as she was supposed to, had dirty clothes left on another bed for over a week, and that call lights took a long time to be answered. A third resident stated there were often only 2 CNAs on dayshift and that on the day with only 1 CNA, call lights were going off for a while waiting for help. Record review and staff interviews confirmed the staffing pattern and the impact on care delivery. The shower documentation showed one resident received showers twice weekly from 6/1/2026 through 7/1/2026, and she later chose to wait for a shower because of storming. Staff stated the facility normally runs with 3 CNAs on dayshift and that 1 CNA was not enough to provide resident care. The report identified that the staffing failure had the potential to affect all 29 residents in the facility.
