Food Served at Below-Appetizing Temperatures

Goldwater Care DanvilleDanville, Illinois Survey Completed on 06-09-2026

Summary

The facility failed to ensure food was served at a preferable temperature for four residents reviewed for dining. On 6/7/26, R3 stated the food was cold and did not taste good. The Resident Council Minutes dated 3/30/26 documented that cold food had been discussed as old news and remained a current resident complaint. The facility’s posted meal times showed breakfast at 7:00 AM, lunch at 12:00 PM, and dinner at 5:00 PM, and on 6/7/26 between 12:00 PM and 1:30 PM room lunch trays were being delivered on an open rolling rack with plates sitting on a warming element and covered, while a dessert item was in a disposable bowl covered with clear wrap. During observation and temperature checks of room-delivered lunch trays, the served food temperatures were below appetizing temperatures. R6’s tray at 1:00 PM showed meat at 99.5°F, vegetable at 98.8°F, and carbohydrate at 97.6°F. R30’s tray at 1:13 PM showed meat at 99.7°F, vegetable at 97.9°F, and carbohydrate at 97.0°F. R8’s tray at 1:15 PM showed meat at 99.8°F, vegetable at 98.5°F, and carbohydrate at 98.0°F. The Week at a Glance listed the lunch meal as Italian Marinated Pork Loin, Stuffing, Roasted Broccoli, Apple Pie, and beverage. At 3:00 PM, the Administrator confirmed the served temperatures were below appetizing temperatures and that tray service was late. The facility’s Dining Options for Meal Service document stated meals would be served no more than 30 minutes after the scheduled meal times.
