Interviews with the Social Services Director and Admission Coordinator confirmed that no PASARR Level II had been submitted, and the Admission Coordinator stated she had never submitted one for the resident despite the documented schizophrenia diagnosis. The resident also expressed feeling isolated and said, "I feel like I am going to go crazy, no one talks to me."
