Infection Control Failures During EBP Care, Equipment Use, and Meal Assistance

Crowley County Nursing CenterOrdway, Colorado Survey Completed on 06-04-2026

Summary

The facility failed to maintain an infection control program in several observed areas, including enhanced barrier precautions, sanitation of vital signs equipment, and hand hygiene during meal assistance. The report identified failures on two halls and in the main dining room, with observations showing staff actions that did not follow the facility’s infection control expectations. For enhanced barrier precautions, an unidentified CNA entered a resident’s room on Hall A and obtained vital signs without donning PPE. On Hall D, an LPN entered the room of a resident on EBP related to an indwelling device without PPE, placed a blood pressure machine beside the bed, applied the cuff with bare hands, touched the resident’s skin, and leaned on the bed rail with a bare arm while the resident took medications. Staff interviews reflected that EBP was understood by some staff to require gown and glove use during direct care, including vital sign collection, but the observed care did not follow that practice. Vital signs equipment was also observed being used without sanitation between residents. A staff member obtained a resident’s blood pressure near the nurses’ station and did not sanitize the machine before or after use. An unidentified CNA used the same machine to take vital signs for a resident on EBP without sanitizing it before entering the room or after leaving. In addition, an LPN removed a pulse oximeter from her pocket and used it on a resident without cleaning it, then returned it to her pocket, and used a stethoscope on another resident without cleaning the diaphragm afterward. Hand hygiene failures were observed during meal assistance in the dining room. One CNA, wearing disposable gloves, fed three residents and did not perform hand hygiene between assisting each resident. During another meal observation, CNA #3 assisted two residents at different times, used the same hand while helping both residents, and did not perform hand hygiene between them until later when she walked to another table and used hand sanitizer. The DON stated staff should perform hand hygiene before assisting residents with meals and between residents if switching from one resident to another.

Failure to Use EBP PPE During Wound Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.

During incontinence care, a CNA removed gloves and applied clean gloves without hand hygiene before placing a clean brief on a resident who was dependent for toileting hygiene and always incontinent. During ostomy care, an LVN handled a resident’s colostomy, wiped stool from the stoma, and continued care without removing soiled gloves, sanitizing hands, or putting on clean gloves. The DON stated staff were expected to perform hand hygiene between glove changes.

Infection Control Failures During Incontinent Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Infection Control Failures During Incontinent Care: Two residents were observed receiving incontinent care with multiple infection control lapses. A CNA did not perform hand hygiene between glove changes or after glove removal while caring for one resident, and another CNA did not wash hands before care, changed nothing between dirty and clean tasks, touched clean items with dirty gloves, placed soiled linens on the floor, and handled dirty linens with bare hands after glove removal. The DON stated hand hygiene, glove changes, and proper handling of soiled linens were required, and facility policies reflected those practices.
