Summary

The facility failed to ensure resident rooms were free from hazardous conditions for two residents. One resident had a history of a prior lumbar vertebra fracture, chronic pain, low back pain, limited lower-extremity movement, and a history of multiple falls. The resident was cognitively intact and used a wheelchair, but was observed standing from a recliner and walking in a hunched, forward-leaning position without using a walker. There were no grip strips on the floor in front of the recliner. The resident also reported that after a prior fall in the bathroom, staff did not come back to help her off the toilet and she fell when she reached for the grab bar and missed. The same resident was later observed asleep in bed without a fall mat at the bedside and with the bed not in the lowest position, despite those measures being included in the fall prevention care plan. There were also two cloth incontinence pads placed on the floor at the bedside. The resident stated she placed the pads there because the floor was cold on her bare feet and said no one told her the pads were unsafe or could lead to falls. Staff acknowledged that the pads on the floor posed a fall risk, that the resident refused the fall mat, and that the resident was placing the pads on the floor. The DON stated the staff had forgotten to apply grip strips in the resident’s new room. A second resident, who was cognitively intact and assessed to need supervision or touching assistance with transfers and walking, had a room observed to be full of clutter. The resident was moving out of the facility, and stacked empty boxes had fallen over. The room contained two empty portable oxygen tanks and multiple items on the floor in the pathway between the bed, bathroom, and exit doorway, including a drill, hangers, clothing, blankets, boxes, and other miscellaneous items. The same cluttered condition was observed on repeated checks over several days. Staff said they had offered to help pick up the items, but the resident refused and did not want anyone touching his things.
