Summary

The facility failed to provide necessary services to maintain personal hygiene for two residents. One resident with dementia, failure to thrive, and a cancer-related wound on the right buttock was assessed as severely cognitively impaired, always incontinent, and dependent on staff for toileting hygiene and perineal care. The resident’s care plan called for checking for incontinence, washing, rinsing, drying the perineum, and changing clothing as needed after episodes of incontinence, and the facility policy stated that staff should use a check-and-change strategy for residents with severe cognitive impairment. During observation, a CNA stated that incontinence care would be provided after breakfast per the resident’s preference, but the resident was observed before breakfast lying in bed and removing a urine-soaked brief with bare hands and tossing it onto the floor. The brief was saturated with urine. Record review showed that perineal hygiene was not documented as completed for a nine- to 13-hour period throughout the day for 14 of 14 days. The resident also had a skin integrity care plan directing staff to keep the skin clean and dry because of the open area on the right buttock. The facility also failed to ensure oral care for another resident who had aphasia, dysphagia, contractures, functional quadriplegia, and an abdominal feeding tube with more than 51% of calories provided by tube feeding. The resident was dependent on staff for all ADLs, including oral care, and indicated that staff had not assisted with brushing or swabbing the teeth on two separate observations. The resident’s teeth were observed to be stained deep yellow with brown and white plaque, with heavy white tartar buildup on the back teeth, and the resident indicated a desire for regular tooth brushing. The DON stated that dependent residents should receive daily oral care, including those who do not take food or drinks by mouth.
