Failure to Maintain RNA Competency Training

Palazzo Post AcuteLos Angeles, California Survey Completed on 06-25-2026

Summary

The facility failed to maintain ongoing competency for restorative nursing assistants (RNAs) by not developing and implementing a restorative nursing assistants training program. During an interview, the Director of Staff Development stated that RNAs weighed residents as part of their job description and daily assignments, and that staff education was provided through written in-service topics posted on the facility’s Annual Staff Development Education calendar. RNA 1 stated that RNAs did weigh residents but could not remember the last time she received education on weighing residents. The Director of Nursing stated that RNAs were required to receive appropriate training to ensure competence in their duties and that weight assessment was one of their primary responsibilities. Review of the facility’s Annual Staff Development Education calendars for 2025 and 2026 showed that the monthly topics did not include weight assessment. Review of the RNA job description showed that RNAs were responsible for providing nursing care using restorative nursing approaches learned through training and actual demonstration, and that their responsibilities included weighing residents monthly and as needed based on facility policy. The facility’s In-service Education Policy stated that ongoing in-service education and training would be provided during orientation, annually, and as needed based on resident care needs or identified competency gaps.
