Inaccurate Admission MDS Documentation for Hearing and Vision Needs
Summary
Licensed nursing staff failed to ensure the admission MDS was accurately completed for two residents by incorrectly documenting hearing aid and vision use. Resident 6 was admitted with diagnoses including COPD, Parkinsonism, and schizoaffective disorder, bipolar type. His MDS indicated minimal hearing difficulty and did not show that he wore a hearing aid, and his care plan also did not identify hearing aid use. However, during observation and interview, Resident 6 stated he wore hearing aids during the day and needed the surveyor to speak louder because he did not hear well. The MDS Nurse later acknowledged the hearing aid information on the MDS was in error and stated that if correct information is not assessed or obtained, staff would not be able to provide proper care needs. Resident 51 was admitted with diagnoses including chronic diastolic CHF, COPD, and pneumonia. His admission inventory listed a hearing aid, and during observation he was seen wearing glasses and stated he could not hear, that he wore a hearing aid, and that the hearing aid needed batteries. The hearing aid was observed in a case on the bedside table. Despite this, the MDS documented minimal hearing and vision impairment but did not indicate use of a hearing aid or corrective lenses, and the care plan also did not identify either item. LN 6 and LN 7 were unaware of the hearing aid use, and the MDS Nurse later acknowledged the MDS was in error because Resident 51 did use a hearing aid and glasses.
