Improper LALM Use for Pressure Ulcer Prevention

Summary

The facility failed to ensure that three residents received pressure ulcer prevention and treatment services in accordance with physician orders, facility policy, and manufacturer guidance for low air loss mattresses (LALMs). Resident 46 had diagnoses including dementia, type 2 diabetes mellitus, and contractures to both knees, and was documented as having severe cognitive impairment, impaired lower extremities, maximal assistance needs for bed mobility, and risk for pressure ulcers. A physician order required a LALM for skin management with the setting monitored every shift, but during observation the Drive-branded LALM was turned on and set at 260 pounds while the resident weighed 81 pounds. The treatment nurse stated the setting was incorrect and too high for the resident’s weight. Resident 113 had diagnoses including dementia, bipolar disorder, and hypertension, and was documented as having severe cognitive impairment, maximal assistance needs for bed mobility, and risk for pressure ulcers. The order listing showed a physician order for a LALM for skin management that had been initiated and later discontinued. During observation, Resident 113 was lying on a Drive-branded LALM that was turned on and blinking low pressure. The treatment nurse stated the mattress should not have been in place because there was no active physician order for it. Resident 117 had diagnoses including dementia, reduced mobility, and hypertension, and was documented as having severe cognitive impairment, maximal assistance needs for bed mobility, and risk for pressure ulcers. The order listing showed a prior LALM order that had been discontinued, and a new order was entered later the same day as the observation. During observation, Resident 117 was lying on a Wellell-branded LALM that was set at 360 pounds while the resident weighed 101 pounds. The treatment nurse stated the setting was incorrect and too high for the resident’s weight. The DON and DSD stated the purpose of the LALM was for skin management and pressure ulcer prevention and treatment, that the setting should match the resident’s weight per manufacturer guidance, and that no documentation was found showing education was provided to the residents or responsible parties about changing the setting against manufacturer guidance.
