Ice Machine Interior Contained Debris

Olympia Convalescent HospitalLos Angeles, California Survey Completed on 07-30-2026

Summary

Food preparation equipment in the kitchen was not kept clean when Ice Machine 1 had yellowish-brown debris inside the ice storage compartment. During a concurrent observation and interview on 7/27/2026 at 7:50 AM, the interior of the ice machine was inspected and a white paper towel used to wipe the compartment where ice is stored picked up scattered wet, yellowish-brown debris. The DSS stated the debris should not have been present inside the ice storage compartment and that the ice machine must be cleaned, possibly more often than once a month. During interviews, the DSS stated the debris could contaminate the ice consumed by residents and that kitchen staff cleaned Ice Machine 1 later that same day after the debris was found. The IPN stated all kitchen equipment should be clean, including the ice machine, and that the machine must be clean to prevent the spread of diseases through water and ice consumed by residents. A review of the 2026 Ice Machine Cleaning Schedule showed Ice Machine 1 was cleaned on 1/13/2026, 2/17/2026, 3/17/2026, 4/27/2026, 5/11/2026, 6/29/2026, and 7/27/2026. The facility policy stated the inside of the ice machine must be washed on no less than a monthly basis, and the FDA Food Code stated equipment food contact surfaces shall be clean to sight and touch.
