Failure to Use EBP and Proper Infection Control During Wound Care

Keystone Post-acuteFresno, California Survey Completed on 07-17-2026

Summary

The facility failed to establish and maintain an effective infection prevention and control program for one sampled resident who had two surgical incision wounds and an ileostomy. During observation, the resident’s room did not have an Enhanced Barrier Precautions (EBP) sign posted on the door, and there was no PPE cart outside or inside the room. The resident stated she had been in the facility for one to two months after surgery and had three surgical wounds, including a wound vac on the abdomen, a dressing on the left abdomen, and an ileostomy bag on the right abdomen. During wound care observation, an LVN performed dressing changes and wound care while using the same gloves across multiple wound and ostomy sites. The LVN removed dressings from the midline abdominal wound, left abdominal wound, and ileostomy site with the same gloves, then changed gloves without performing hand hygiene before donning new gloves. The LVN used gauze from the same saline cup to clean the left abdominal wound, the ileostomy site, and the middle wound without changing gloves or performing hand hygiene between sites. The LVN also touched the open ileostomy site with the edge of the wound vac dressing while preparing the dressing, and no new gloves were applied after that contact. Record review showed the resident was cognitively intact and had diagnoses including malignant neoplasm, ileostomy, acute kidney failure, ovarian neoplasm, anxiety disorder, perforation of intestine, and right lower quadrant pain. The resident had no order for EBP and no care plan for EBP, and staff stated she was not on EBP. Facility staff, including the wound nurse, infection prevention staff, and DON, stated the resident should have been on EBP because she had wounds and an ostomy. The facility’s EBP policy stated residents with wounds, including unhealed surgical wounds, and indwelling medical devices should be placed on EBP, with gown and glove use during high-contact care activities and clear signage posted outside the room.

Infection Control Practices Not Consistently Implemented for Resident Evaluated for C. difficile
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident being evaluated for C. difficile was not consistently managed under the correct contact-enteric precautions. Staff gave care with incomplete understanding of the precautions, the room signage did not clearly identify the needed disinfectant or contact time, bleach wipes were not always available, and staff were observed missing hand hygiene and handling items under PPE during care.

Failure to Use EBP PPE During Wound Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.

Failure to Use Required PPE for Resident on Contact Precautions
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident on Contact Precautions for active C. difficile infection was not consistently protected by required PPE. Although signage outside the room directed all entrants to wear a gown and gloves, an RN entered the room with medications without either item and touched the bedside table and door surface. The resident stated staff did not always wear gowns, and the DON confirmed staff were expected to wear a gown and gloves when entering the room while precautions were in place.

Infection Control Lapses During Incontinence and Ostomy Care
E
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A facility failed to maintain infection control during care for two residents. During incontinence care, a CNA removed gloves and applied clean gloves without hand hygiene before placing a clean brief on a resident who was dependent for toileting hygiene and always incontinent. During ostomy care, an LVN handled a resident’s colostomy, wiped stool from the stoma, and continued care without removing soiled gloves, sanitizing hands, or putting on clean gloves. The DON stated staff were expected to perform hand hygiene between glove changes.

Infection Control Failures During Incontinent Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Infection Control Failures During Incontinent Care: Two residents were observed receiving incontinent care with multiple infection control lapses. A CNA did not perform hand hygiene between glove changes or after glove removal while caring for one resident, and another CNA did not wash hands before care, changed nothing between dirty and clean tasks, touched clean items with dirty gloves, placed soiled linens on the floor, and handled dirty linens with bare hands after glove removal. The DON stated hand hygiene, glove changes, and proper handling of soiled linens were required, and facility policies reflected those practices.
