Ineffective pest control with cockroaches in kitchen and flies in resident areas
Summary
The facility failed to maintain an effective pest control program when cockroaches were observed in the kitchen and throughout the building. During observation, a cockroach was seen crawling on the floor toward the tray line table near the kitchen tray line, and another cockroach was seen crawling under the tray line table during lunch service. A bug trap under the tray line table contained more than 25 cockroaches. The Certified Dietary Manager stated cockroaches had been observed throughout the kitchen for approximately a year, and the Maintenance Director confirmed that cockroaches had been present in the kitchen since 10/23/25 based on the pest control service log. The pest control logs also documented cockroaches in resident rooms, offices, and throughout the facility. Kitchen audits noted pest control was not met and that traps needed to be constantly changed. The Diet Type Report showed 64 of 65 residents received food from the kitchen. Residents and staff also reported flies in resident areas and common areas. In one shared room, a fly was observed while a resident with an open wound on the left ankle and a history of osteomyelitis and other diagnoses was lying in bed holding a fly swatter. The resident stated flies were often in the room because staff left doors open and said staff had given her the fly swatter to get the flies. In another room, a resident with wounds on the abdomen, a wound vac, and an ileostomy had a fly observed leaving the room. A second resident had a fly swatter in a container with personal items on the bedside table over the bed and stated flies and mosquitoes were present in the room, especially in the evening. A fly was also observed in the hallway between the kitchen and dining room while the Activities Director was present, and the AD stated flies came in when the outside door was opened and that flies had been a problem in the facility. The AD stated there were no bug zappers or fly traps in the facility. Staff interviews reflected that flies and roaches had been seen in resident rooms and throughout the facility, that flies were not sanitary, and that pests could contaminate resident items and wounds. The DON stated the expectation was for a pest-free environment and acknowledged that the current pest control program was not effective. The facility policy described a safe, clean, comfortable, and homelike environment, and the pest control scope of work called for a clean, safe, and pest-free environment with additional on-call services as needed.
