Summary

The facility failed to develop and implement a comprehensive person-centered care plan for one resident's fall risk. The resident was admitted with diagnoses including difficulty walking, a prior fall, dementia, generalized muscle weakness, and dizziness. The admission fall risk evaluation identified one to two falls in the prior three months, intermittent confusion, poor vision, and a fall risk score of 12, which indicated high risk for falls. The history and physical noted the resident was cognitively impaired and required monitoring for safety functions, and the MDS later documented moderately impaired cognition and the need for supervision with showering. After a change of condition assessment found the resident on the floor sitting upright, the fall risk evaluation was updated and showed a score of 8. The care plan initiated afterward included interventions such as floor mats, placement close to the nursing station, and frequent visual monitoring, and later care plans continued to list floor mats along with frequent visual monitoring and the falling star/superstar program. During interview and observation, the resident stated she could walk on her own but did get dizzy, and no landing mats were observed in the room. The DSD and DON stated the resident did not have a landing mat, that floor mats were contraindicated because the resident ambulates and they would be a trip hazard, and that the care plan was not accurate or resident-centered because it listed an intervention that would not be applicable to the resident.
