Summary

The facility failed to respect the personal privacy of one resident when a CNA entered the resident’s room without permission while the resident was being changed. Resident 2 was admitted on 5/9/2017 and readmitted on 9/17/2025 with diagnoses including MI, COPD, and type 2 DM. The resident’s MDS dated 5/28/2026 indicated intact cognition and that the resident required maximal assistance from staff with toileting hygiene, showering or bathing, dressing, and toilet transfer. During an interview, Resident 2 stated that on 7/10/2026 before 7 a.m., while CNA 9 was changing her incontinence brief, CNA 7 walked into the room without knocking. Resident 2 stated she told CNA 7 to leave because CNA 7 was not assigned to her and she did not feel comfortable having CNA 7 in the room during the care. CNA 7 later stated she entered the room without the resident’s permission and followed CNA 9 to help at the end of the shift, and acknowledged she should have asked permission before entering. The ADON also stated CNA 7 should have asked permission because it is the resident’s right to have personal privacy and space respected.
