Summary

The facility failed to ensure that the environment remained as free of accident hazards as possible when a resident's bed was left in the raised position and unattended. The resident had been admitted and later readmitted with diagnoses including an unspecified head injury, anoxic brain damage, and epilepsy. The resident's MDS indicated dependence on caregivers for all daily activities and limited range of motion in all four limbs, and the H&P noted cognitive impairment. The care plan identified the resident as at risk for falls and injury related to impaired cognition, poor body balance and control, poor safety awareness and judgement, seizure disorder, and general weakness, with floor mats and padded side rails used to prevent or reduce injury and falls. During observation, a CNA left the resident's bed raised after working with the resident and then left the room without returning. A subsequent observation with an LVN found the bed still not in the low position, and the LVN stated the bed should be in the low position for safety reasons but left it raised because the resident was not at risk for a fall since padded railings were in place. Later, a DSD lowered the bed and stated it should be kept in the lowest position as part of standard fall precautions to prevent injury. An RN and the ADON also stated that keeping the bed in the lowest position was standard fall prevention, and the ADON stated that if a bed was left in the raised position, there was a risk of falls. The facility policy stated that caregivers should always make sure beds are in the lowest position to the floor and that wheels are locked.
