Summary

The facility failed to provide a notice of transfer to 51 long-term care residents and/or their representatives at least 30 days before transfer. The record review showed that each of the 51 residents was transferred to another facility within 10 days of receiving transfer notification, rather than receiving advance notice 30 days before the move. The residents had varying admission histories, including both original admissions and readmissions, and all were moved from the facility in July 2026. Several resident interviews reflected distress related to the transfers. Resident 37 had a BIMS score of 15 out of 15 and stated that the transfer to a facility 65 miles away was very stressful and could interfere with weekend visits with a friend. Resident 51 also had a BIMS score of 15 out of 15 and, along with a spouse, expressed distress because the move would make daily visits difficult due to lack of transportation. Resident 33, who also had a BIMS score of 15 out of 15, cried while stating that the receiving facility was 100 miles away and that family lived nearby. For Resident 1, who had a BIMS score of 11 out of 15 with mild memory impairment, the representative reported receiving a phone call from the resident on 7/14/26 after being told a transfer would occur, followed by a closure letter three days later, and the resident was transferred on 7/17/26. The facility’s undated closure letter stated that residents, families, and representatives were being notified because of roof issues requiring immediate attention and that residents would be temporarily relocated over approximately the next two weeks. The Administrator confirmed that the closure letters were mailed on 7/14/26. The facility policy titled Transfer and Discharge (including AMA), revised 5/2026, stated that notice should be provided at least 30 days prior to a facility-initiated transfer or discharge.
