Summary

Professional standards of quality were not followed for four residents when nursing staff failed to carry out ordered medication and pain-management practices as documented in the record and observed by surveyors. Resident 3, who was admitted with chronic pain syndrome and pain in the thoracic spine and who had multiple pressure-related wounds, was observed during wound care moaning, yelling, and stating severe pain. The record showed orders for scheduled morphine, PRN acetaminophen, and documentation of pain each shift, yet surveyors found repeated missed opportunities to administer acetaminophen, no documented evidence that it was offered, refused, or given, and no pain assessment or pain medication provided before wound treatment. The DON confirmed that pain should be assessed and medication given before care when possible, but during the observed wound treatment the resident continued to yell in pain and oxycodone was not administered until after the treatment had ended. Resident 12, who had diagnoses including seizures, depression, and schizoaffective disorder, had a physician order for melatonin at bedtime for inability to sleep. The record review with the DON showed that psychiatry documented concern for altered mental status and sedation and recommended discontinuing melatonin, and the physician note reflected that the order should have been discontinued. However, the medication remained on the physician's orders and had not been discontinued. Resident 4, who had type 1 diabetes, muscle weakness, and dysarthria following cerebral infarct, had an order for oxycodone 5 mg via PEG tube every 8 hours as needed for severe pain with a pain score of 7 to 10. Review of the June and July MARs showed nursing staff administered oxycodone for pain scores outside the ordered parameters, including pain levels of 5, 6, and 0. Resident 1, who had type 2 diabetes, had an order for semaglutide 14 mg daily 30 minutes before breakfast with instructions to hold the medication if blood sugar was less than 100 mg/dl. The DON confirmed the medication was given daily at 5:45 a.m., but blood sugar was not routinely checked before administration to verify the hold parameter was met.
