Infection Control and AC Filter Maintenance Deficiencies

Summary

The facility failed to maintain an effective infection prevention and control program when staff did not follow contact precautions for three residents who were on contact precautions for infectious disease-related conditions and multidrug-resistant organisms. Resident 15 had diagnoses including serious breathing problems, a history of infections, resistance to multiple antibiotics, and a stage 4 pressure injury. Resident 58 had diagnoses including stroke, ventilator dependence, and carrier status for a bacteria resistant to strong antibiotics. Resident 61 had diagnoses including stroke, urinary tract infection, resistance to multiple antibiotics, and carrier status for a bacteria resistant to strong antibiotics. Their care plans directed staff to follow contact precautions and PPE protocols. During a concurrent observation and interview, a CNA was observed inside the shared room of these residents without a gown. The CNA wore gloves, picked up three urinals filled with urine in one hand, exited the room, walked to the shower room with the urinals, returned with the empty urinals, and re-entered the room without sanitizing hands or putting on a gown. The CNA placed the empty urinals at each bedside, emptied the trash, and took the trash outside the room. The CNA confirmed the observations and stated she knew she was supposed to wear a gown but was in a rush. The Infection Preventionist stated residents on contact precautions required gown and gloves during care, that PPE should be worn before entering the room, and that the CNA should not have carried all three full urinals in one hand or re-entered the room without proper PPE and hand hygiene. The facility also failed to maintain portable AC unit filters in resident rooms. Two portable AC units were observed with visibly soiled filters heavily saturated with dark debris, and a family member stated the filter in one room had not been cleaned. The maintenance director confirmed the filters were heavily soiled and obstructed, affecting the units' ability to circulate air effectively, and stated the filters needed to be cleaned weekly but had not been serviced for a month because he had been on vacation and had no one to delegate the task to. The weekly AC filter cleaning log showed missing entries, and 33 resident rooms housing 77 residents were affected.
