Summary

The facility failed to keep confidential resident health records secure when not in use during a medication pass for a census of 92 residents. During observation in Hall C, a Licensed Nurse was preparing medications for one resident while another resident’s phenytoin suspension medication box with a pharmacy label was left on top of the med cart. The nurse also removed an eye drop from its manufacturer’s box for administration and set that box on top of the med cart. Both boxes contained pharmacy labels with confidential resident information, and the nurse entered the resident’s room to administer medications while the boxes remained unattended and visible to staff and residents passing by. During a later observation the same morning, the phenytoin suspension box was again seen unattended on the med cart. In interview, the Licensed Nurse confirmed she had left confidential resident information accessible and unsecure when the labeled medication boxes were left unattended on the med cart during the morning med pass. The DON stated it was not acceptable to leave resident medication information unattended on the med cart. The facility’s policy stated that resident personal and medical records are to be kept confidential and safeguarded from unauthorized disclosure.
