Medication Access, Labeling, and Disposal Deficiencies

Summary

The facility failed to ensure that unauthorized nursing staff no longer had access to the Automated Drug Dispensing System after their employment ended. During observation and interview with the DON in the medication storage room, the ADDS was reviewed and a list of authorized nursing staff was displayed. The DON identified two names on the list and stated those nurses were no longer employed by the facility. The DON also stated she had not followed up with the pharmacy to terminate their access and that there was no process in place to ensure access was removed in a timely manner. The facility also failed to ensure medications and biologicals were labeled in accordance with accepted professional principles in medication carts A and D. In med cart A, several inhalers and one Novolog vial were found in manufacturer boxes with pharmacy labels on the boxes, but without resident-specific labeling on the medications themselves. In med cart D, two budesonide/formoterol inhalers were found in labeled boxes without resident-specific labeling on the inhalers, one Breyna inhaler was in a box without a pharmacy label and had a room number written on it in blue marker, and three sterile saline vials for inhalation were found in the drawer without a pharmacy label. Staff stated the practice was to keep the medications in the labeled boxes so the residents could be identified. The facility also failed to keep refused medication doses out of the med cart. An LPN was observed leaving a resident’s room with a medicine cup containing pills after the resident refused the medication. The LPN placed a second cup over the pills, wrote the room number on it, and placed the cups in the top drawer of the med cart for later disposal. The ADON and DON both stated refused medications were expected to be destroyed immediately and not stored in the med cart, and the ADON stated the practice was also an infection control concern because the medication had entered and exited the resident’s room.
