Summary

The facility failed to ensure informed consent was obtained before psychotropic medications were administered to three sampled residents. Resident 2 had a physician order for duloxetine delayed release Sprinkle 60 mg daily for depression, but during a concurrent interview and record review with the DON, no informed consent for duloxetine was found. Resident 20 had a physician order for Lamictal 50 mg twice daily for bipolar disorder, and the DON confirmed no informed consent was present for Lamictal, stating that nursing staff were expected to ensure informed consent was obtained by the doctor before the first dose of a psychotropic medication was given. Resident 30 had physician orders for multiple psychotropic medications, including citalopram 10 mg daily for repetitive counting and clapping related to unspecified psychosis, Depakote 125 mg two capsules twice daily for agitation related to depression, quetiapine 25 mg daily for generalized anxiety disorder related to unspecified psychosis, and trazodone 50 mg at bedtime for inability to sleep related to depression. During review with the DON, no informed consent was found for citalopram, Depakote, or quetiapine. The DON stated that trazodone had an informed consent, but it was incomplete because the dosage was not indicated and the dosage and frequency were expected to be documented on the consent. The facility policy stated that prior to initiating or increasing a psychotropic medication, the resident, family, and/or resident representative must be informed of the benefits, risks, and alternatives, and the facility must document that this information was provided.
