Failure to Assess and Report Resident Skin Changes
Summary
The facility failed to ensure treatment and care were provided in accordance with professional standards for one resident with dementia, hypertension, atopic dermatitis, and bullous pemphigoid. The resident’s records showed severe cognitive impairment and dependence for many activities of daily living. Dermatology documentation described chronic skin involvement with urticarial lesions, large tense vesicles, erosions, and denuded surfaces affecting the head, neck, chest, abdomen, back, pelvis, and extremities, and the care plan identified altered skin integrity related to bullous pemphigoid and recurrent bullous pemphigoid with interventions to monitor for deterioration and infection. During observation, the resident was seen with generalized scabbing of varying sizes on the upper chest, neck, and hands, with some areas dry and others open. Later, the resident was observed scratching the right leg and face, and a CNA reported frequent scratching with wounds on the right side of the body, upper arms, lower legs, neck, and lower chin. The record review and interviews showed that the nursing documentation did not specify when the skin condition began, and the LVN stated that any changes in skin should prompt a change of condition assessment. The LVN also stated she was unfamiliar with the proper documentation procedure for wound assessments. The DON reviewed the wound management and change of condition documentation and was unable to provide a nursing assessment showing whether the resident’s skin condition was improving or deteriorating. The DON stated there was no record of assessment or monitoring of wound progress and that the resident’s skin changes should have been monitored, documented weekly, and reported to the physician and resident representative. The DON further stated that the documentation could not show when the scab or open wound in the neck or chin area first developed, that a change of condition evaluation was not initiated, and that the resident’s representative was not notified of the progression of the skin condition to the facial area. The resident’s family member also stated there had been a lack of communication about the progression of the condition to the face and the resident’s current state.
