Bathroom Disrepair Prevented Safe and Homelike Use
Summary
The facility failed to provide a safe, clean, comfortable, and homelike environment for four residents who shared a bathroom between two rooms. During observation and interview, the bathroom was found to have a toilet that moved side to side, missing and cracked caulking, a thick black substance around the base and in the corners, a chipped wall, and a linoleum floor that was stained yellow and brown and peeling. A commode had been placed over the toilet, but the toilet seat on the commode slid when used, and the commode was described as too high for some residents and not meeting their safety needs. Resident 1, who had a BIMS score of 15, stated she had complained about the bathroom many times and did not feel safe using it. Resident 2, also with a BIMS score of 15, demonstrated that the commode seat slid when she sat down and got up, which made her nervous. Resident 3, who had a stroke and a BIMS score of 9, stated she needed help getting on and off the toilet, could not get her wheelchair into the bathroom because there was not enough room, and showed scuff marks and chipped wall areas caused by the wheelchair. Resident 4, with a BIMS score of 11, stated she was in pain, needed help getting to the bathroom, and said the commode was too high and hurt her when she used it. Staff and management confirmed the condition of the bathroom. The DSD, CNAs, HK, LVN, MD, and Administrator all observed or discussed the bathroom and described it as not appealing, gross, unsafe, not sanitary, and not homelike. The MD acknowledged the toilet still moved, the caulking was still missing in places, the floor was stained and old, and the commode did not meet residents' safety needs. Facility policies stated residents are to be provided a safe, clean, comfortable, and homelike environment and that maintenance is responsible for keeping the building and equipment in safe, operable, and good repair.
