Unlabeled and Expired Food Items in Kitchen and Resident Room

Longwood Manor Conv.hospitalLos Angeles, California Survey Completed on 06-26-2026

Summary

Food items were found not labeled and, in some cases, expired during observations in the kitchen. In the walk-in refrigerator, one pitcher of cranberry juice and one gallon of hamburger dill slices were not labeled, and a 32-ounce container of minced garlic had an expiration date of 5/15/2026. In the walk-in freezer, an opened bag of chicken patties was not labeled. On the spice rack, one pound of opened ground ginger and a 16-ounce lemon juice bottle were also not labeled. The Dietary Services Supervisor stated that not labeling food items could prevent staff from identifying the items or knowing whether they were expired, and that failing to discard expired food could result in foodborne illnesses. The facility policy titled Dating and Labeling required items to be properly covered, dated, labeled, and removed when expired or beyond the best buy date. Food items in one resident's room were also found not dated and labeled. Resident 20 was admitted and readmitted to the facility, had diagnoses including COPD, emphysema, and schizophrenia, and was documented as having the capacity to understand and make medical decisions with intact cognition. The resident's MDS indicated supervision was needed for personal hygiene, showers, and dressing. During observation, an opened jar of peanut butter and mayonnaise in the resident's room were not dated or labeled. The resident's IDT note documented that the resident was hoarding food items in drawers and on the bedside table. MDS staff and the ADON stated the food items should have been labeled and dated, and that the opened food items in the room had the potential to become contaminated and cause food borne illness. The facility policy on food from outside sources stated that outside food brought in by visitors and family was permitted and that staff would assist with safe and sanitary storage, with all outside foods properly dated and labeled.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
