Daily Nurse Staffing Information Not Updated and Posted

Primrose Post-acuteInglewood, California Survey Completed on 07-10-2026

Summary

The facility failed to post the updated daily nurse staffing information at the beginning of the day. During a concurrent observation and interview on 7/7/2026 at 8:50 a.m. at Station A, the staffing information posted was dated 7/6/2026. The Infection Preventionist Nurse stated that the nurse staffing information for 7/7/2026 had not been posted and updated, and explained that the posting should include the projected and actual hours worked by licensed nurses and CNAs providing direct care, along with the actual total number of residents living in the facility. The Infection Preventionist Nurse stated the daily nurse staffing information should be prepared the day before and posted before the beginning of the shift, and updated daily so residents, staff, and visitors would know whether there were enough staff present to provide care and accommodate residents' needs. During an interview on 7/8/2026 at 2:40 p.m., the DON stated it was important for the nurse staffing information to be updated and posted daily to show compliance with regulations and to show how many staff the facility planned based on the census to care for and manage the residents living in the facility. A review of the facility's undated policy titled, Posting Direct Care Daily Staffing Numbers, indicated the facility would post on a daily basis for each shift the number of nursing personnel responsible for providing direct care to residents.

F0732 F732: Post nurse staffing information every day.
Short Summary

Failure to Post Required Daily Staffing Information: The facility did not post the required daily census and actual DHPPD information for the prior day, even though only the next day’s estimated staffing form was displayed in the posting area. The ADM said the Scheduler posted the forms before leaving, but the Scheduler stated she had only been instructed to post the next day’s estimated form, not the prior day’s actual census and DHPPD data. The DON confirmed the required prior-day form was not posted, despite the facility policy requiring daily staffing postings in a prominent location accessible to residents and visitors.

Inaccurate daily nurse staffing posting
D
F0732 F732: Post nurse staffing information every day.
Short Summary

Inaccurate daily nurse staffing information was posted in the window at the main entrance and did not reflect the actual resident census or the hours worked by RNs, LPNs, CMAs, and CNAs. Staff said receptionists completed the posting each morning, used the schedule inconsistently, did not know which nurses were RNs or LPNs, and did not update the posting as staffing changed. The facility had no policy for the posting, and the Facility Assessment called for staffing to be adjusted based on census and resident care needs.
