Summary

The facility failed to complete the comprehensive admission assessment for one sampled resident after admission. Resident 1 was admitted with multiple diagnoses, including hemiplegia and hemiparesis following cerebral infarction, type 2 diabetes mellitus, hypertension, gastrostomy, dysphagia, aphasia, difficulty walking, protein-calorie malnutrition, atrial fibrillation, gastrointestinal hemorrhage, muscle weakness, and urinary retention. During record review and interview with the MDS Coordinator, the resident’s admission assessment remained in progress and had not been completed by the required completion date. The MDS tracking screen showed the assessment was 12 days overdue, and the MDS Coordinator confirmed it was not completed on time. The MDS Coordinator stated that when the admission comprehensive assessment is not completed, it delays identification of the resident’s care needs and the implementation of appropriate interventions because the CAAs and comprehensive care plan are not completed. The ADON also stated that resident assessments were expected to be completed timely because they help identify care needs and develop appropriate interventions in the care plan.
