Failure to Implement Fall Prevention Measures and Supervision
Summary
The facility failed to ensure two residents were free from accidents and hazards when fall risk assessment and fall-prevention interventions were not implemented as documented. One resident had diagnoses including hemiplegia, hemiparesis following cerebral infarction, muscle weakness, and reduced mobility, and had a BIMS score of 15. After an unwitnessed fall, the resident’s fall risk assessment recorded a score indicating recognizable fall risk factors, but the assessment also marked no falls in the past 3 months even though a prior fall had already occurred. The record also showed a care plan entry for an actual fall with toileting schedule as an intervention, and later interdisciplinary notes stated a bedside commode was to be placed within reach. However, the medical record lacked documentation showing that a toileting schedule or bedside commode was implemented before the resident’s later unwitnessed fall while being assisted to the bathroom. After that later fall, the resident was found to have a displaced fracture of the proximal humerus. The resident stated that a CNA assisted her to the bathroom and then left, and while she was trying to come out of the bathroom her legs gave out and she fell. The DON and LVN confirmed the resident had fallen before the fall risk assessment and acknowledged the resident was not accurately assessed. They also confirmed there was no documentation that the toileting schedule or bedside commode had been implemented, and the DON stated the CNA should not have left the resident alone in the bathroom because the resident required assistance for ambulation. OT documentation also indicated the resident required supervision or touching assistance for toilet transfer, walking 10 feet, and toilet/commode transfers. A second resident had diagnoses including CKD, epilepsy, muscle weakness, and gait and mobility abnormalities, with a BIMS score of 15. The resident experienced multiple falls in June, and the fall care plan repeatedly listed pharmacy consultation and PT consultation as interventions. An interdisciplinary note after one fall described lower-extremity weakness, balance impairment, and possible orthostatic hypotension or neuropathy, and listed PT/OT evaluation and orthostatic BP monitoring as new or revised interventions. The record review and DON interview showed these interventions were not documented as implemented in a timely manner, and PT consultation documentation was not present until after one of the falls. Following the resident’s unwitnessed fall, the resident was transferred to the hospital and CT results showed a fracture involving the anterior aspect of C1. The care plan later included 1:1 monitoring for safety tracking and monitoring, but during observation the resident was sitting alone without staff providing 1:1. Staff interviews confirmed the assigned CNA had gone on lunch break without informing the LVN, and the DON confirmed there was no documentation that the pharmacy consult or orthostatic BP checks had been implemented and that PT/OT consultation had not been timely performed for the resident’s repeated falls.
