Kitchen sanitation and food handling deficiencies
Summary
The facility failed to ensure food preparation equipment and the kitchen environment were maintained in a clean and sanitary condition during meal service. During a kitchen tour, a brown dry substance was observed on the refrigerator floor, a plastic container of peaches in the refrigerator did not have an opened date, and a bag of corn dogs and garlic sticks in the freezer was uncovered and had frost on it. The Dietary Manager later stated the brown substance appeared to be cardboard box debris that could collect contaminants and build up mold, and that the open frozen food needed to be disposed of because it was open to air and had frost on it. During meal preparation observations, staff were seen handling food in ways that did not maintain sanitation. One staff member’s oven mitt touched the inside of cooked pork while removing it from the oven, and while transferring pureed Brussel sprouts, food splashed onto the staff member’s shirt. The staff member used a finger to remove food from the shirt, placed that food on aluminum foil, and then used the same finger to remove splashed sprouts from the pan and place them on the foil without hand hygiene. Another staff member removed sweet potatoes from the oven, took off oven mitts, and then, without hand hygiene, removed an aluminum foil lid and touched the mixer lid with fingers that had contacted the inside of the lid. A cook also wiped the workstation with a sanitizer cloth and inserted the mixing blade into the mixer without sanitizing hands. Additional observations showed other food handling and hygiene lapses. A Dietary Aid pressed pie crust into pans with gloved hands and then used the same gloves to open a new package of pie crust and press it into another pan. A staff member inserted a thermometer probe into broccoli past the metal probe, touching the plastic portion near the display screen, and did not fully wipe the thermometer before using it on other foods, leaving broccoli particles on other items. Another staff member did not wash hands before meal prep and handled plates so the palm touched the food surface. Loose hair was also observed outside the hair nets on multiple dietary staff. Interviews confirmed staff expectations for handwashing, thermometer cleaning, and hair containment, and facility records showed prior in-services on handwashing, food handling, and hair coverage.
