Care Plans Not Kept Current for Resident Meetings, Code Status, and Oxygen Orders
Summary
The facility failed to keep comprehensive care plans current for three sampled residents and did not ensure care plan conferences and revisions reflected their current needs. The report states that the comprehensive care plans were not updated, care plan conferences were not conducted as expected, and the plans were not revised to match current resident conditions. The deficiency was identified through clinical record review, staff interviews, resident/family interviews, and review of facility policy. For one resident, the record showed an admission care plan meeting and one quarterly care plan meeting, but the resident stated she could not recall being part of care plan meetings. Her daughter stated the facility had not had a care plan meeting this quarter and had not had one in a while, and that there had been no communication from the facility about why meetings were not held. The administrator and DON reviewed the record and confirmed only two care plan meetings were documented since admission, and the DON stated she expected care plan meetings to be held quarterly. For another resident, the physician had ordered DNR status, but the comprehensive care plan still listed the resident as Full Code. Staff reviewing the chart confirmed the DNR order and the completed DNR form in the Go Book, but also confirmed the care plan had not been updated. The DON acknowledged the care plan should have been revised to reflect the resident's current code status and that it had not been changed after the DNR order was entered. For the third resident, the physician orders changed the oxygen order to continuous oxygen at 4-8 liters per minute via nasal cannula, but the care plan and Kardex still listed 4-6 liters per minute. During observation, the resident was on 8 liters of oxygen. Staff interviews confirmed that the Kardex and care plan were not updated to match the current order, and the ADON stated both documents should have been current. The progress notes also showed an interdisciplinary care conference note stating the resident remained on 6 liters per minute with oxygen saturation dropping with activity, while the current order allowed a higher range.
