Summary

The facility failed to ensure that a physician order for a wheelchair cushion was followed for a resident with severe morbid obesity, metabolic encephalopathy, type 2 diabetes mellitus, acute respiratory failure with hypoxia, pneumonitis, malignant neoplasm of the pancreas, and age-related physical debility. The resident was documented as bedfast most of the time, very weak, severely debilitated, and at high risk for pressure ulcer development. The care plan included interventions for turning and repositioning as tolerated and use of a pressure-reducing device in the wheelchair, and a physician order dated July 12, 2026, directed that a wheelchair cushion be used every shift. Therapy documentation showed the resident required substantial to maximal assistance for bed mobility and transfers.
