Incomplete Dialysis Communication and Documentation
Summary
The facility failed to ensure that a resident who required dialysis received safe, appropriate dialysis-related care and that communication, coordination, and collaboration with the dialysis provider were maintained. Resident #1 was a cognitively intact male admitted after hospitalization for acute on chronic renal failure, end stage renal disease, acute metabolic acidosis, and metabolic encephalopathy. He had a hemodialysis catheter placed during the hospitalization and was receiving hemodialysis three times per week. His care plan identified risks related to dialysis, including shortness of breath, chest pain, blood pressure changes, itching, nausea and vomiting, impaired cognition, and infection at the shunt site, and directed staff to monitor his condition before and after dialysis and report abnormalities to the MD. Record review showed that only two hemodialysis communication forms were available in the facility system for the resident, despite his stay being longer than one month. The forms reviewed were for dialysis visits on 05/19/26 and 06/11/26. On both forms, the pre-treatment vital signs were documented, but the post-treatment temperature, pulse, respirations, blood pressure, and weight sections were left blank. The 05/19/26 form included dialysis center documentation of excessive fluid weight gain, a mild restriction of one liter of fluid intake, review of blood test results, and a request to review the medication list and phosphate binder orders with meals. The 06/11/26 form included dialysis center documentation of fluid overload, fluid restriction of 1.5 liters per day, and dialysis treatment information. Neither form contained documentation showing facility follow-up regarding the dialysis center’s recommendations. The facility’s PCC record review showed no scanned hemodialysis communication forms other than those two appointments and no nursing notes indicating staff contacted the dialysis center to obtain missing information. Interviews with LVN A, LVN B, and the DON confirmed that staff were expected to complete the dialysis communication form before the resident left for dialysis and again upon return, document post-dialysis vital signs and assessments in PCC, and contact the dialysis center if information was incomplete or missing. The DON also confirmed that the facility should have documented vital signs and assessments in PCC after each dialysis appointment. The dialysis center RN Admin stated the facility had not contacted the dialysis center about missing dialysis communication information, and that the forms were expected to document treatment information, vital signs, fluid removal, medication changes, physician recommendations, changes in condition, and follow-up needs.
