Summary

The facility failed to ensure CR#1’s assessment accurately reflected his status when the initial MDS did not code him as having an indwelling Foley catheter. CR#1 was a male resident admitted with diagnoses including hypertension, hyperlipidemia, chronic kidney disease stage V, and acute kidney failure. His physician’s assessment included an order for bladder scans and insertion of a Foley catheter if urine was greater than 300 ml, and bladder scans documented 375 ml on 5/18/2026, with additional scans showing 10 ml, 100 ml, 0, and 150 ml on subsequent dates. Record review showed hospital notes dated 5/26/2026 documented hematuria in CR#1’s Foley bag and imaging that demonstrated malposition of the Foley catheter, with the Foley replaced in the ER and symptoms resolving afterward. However, the facility’s initial MDS coded CR#1 as frequently incontinent for bowel and bladder and did not indicate an indwelling catheter. There was no physician order in the May 2026 record for Foley insertion or Foley care, and progress notes between 5/18/26 and 5/26/2026 did not document that a Foley catheter was inserted while he was at the facility. During interview, the ADON and MDS Coordinator reviewed the record and stated they did not see documentation of Foley insertion, and the MDS Coordinator said the MDS was not coded because there was no order for a Foley.
