Infection Control Lapse During Wound Care

Summary

The facility failed to maintain an infection prevention and control program when the wound care nurse did not perform hand hygiene and glove changes appropriately during wound care for two residents with wounds. One resident was a female admitted with diagnoses including peripheral vascular disease, anemia, hypertension, aneurysm of the artery of the lower extremities, and multiple sclerosis, and she had a surgical incision to the groin with orders for daily wound care. During observation of wound care, the nurse removed the old dressing, changed gloves and washed hands, cleaned the wound, and then applied the clean dressing without changing gloves or washing hands between the dirty and clean portions of the procedure. A second resident was a male admitted and readmitted with diagnoses including sepsis, hypertension, benign prostatic hyperplasia, and one unhealed pressure sore; he was coded as cognitively aware with a BIMS score of 14 and was incontinent of bowel and bladder. His physician’s order directed daily treatment to the right heel wound. During observation of wound care, the nurse washed her hands, put on gloves, removed the soiled dressing, washed her hands again, put on clean gloves, and cleaned the wound, but then dried the heel and applied the dressing without changing gloves or washing her hands between cleaning and dressing the wound. During interview, the wound care nurse stated she should have changed gloves between cleaning the wound and applying the dressing and said not doing so could result in infection or spread of infection. The regional nurse stated the expectation was for nurses to change gloves and wash hands between dry and clean procedures during wound care. The facility’s wound care procedure also described hand hygiene and glove changes between removal of the old dressing and cleaning of the wound, and again after wound care before repositioning the resident and handling supplies.
