Incomplete Annual Staff Training
Summary
The facility failed to maintain an effective training program for all new and existing staff members when record review showed that CNA A and Dietary Aide E did not have required annual training completed for multiple subjects. On 05/06/2026, the HR Coordinator provided personnel file information for 7 sampled employees, which showed CNA A, hired in December 2024, was missing training in communication, infection control, compliance and ethics, HIV, restraint reduction, and prevention of falls. Dietary Aide E, hired in December 2023, was missing training in communication, infection control, HIV, and restraint reduction. During interview, the HR Coordinator stated she did not have documentation of completed trainings for CNA A and Dietary Aide E and identified the missing subjects as communication, HIV, infection control, compliance and ethics for CNA A, restraint reduction, and fall prevention for CNA A. She stated it was not acceptable for staff to be working without completed annual training and explained that annual training was needed so staff would be refreshed and competent on facility regulations and policies. She also stated that department heads were responsible for ensuring employees were up to date on training. The DON stated that a daily morning meeting included a printed list of employees with missing trainings who were not in compliance, and that employees with incomplete training for more than a month would be relieved of duties until completion. The Administrator stated that Corporate HR was responsible for unlocking and scheduling trainings in the software, while the facility monitored completion through daily review of scheduled training reports. Record review of the training schedules showed no completed trainings on file for the subjects reviewed for either CNA A or Dietary Aide E, and the facility’s Employee Handbook stated that ongoing and annual training would be conducted and retained in the employee record.
