Improper Storage of Dentures and Oxygen Equipment

Avir At El PasoEl Paso, Texas Survey Completed on 05-06-2026

Summary

The facility failed to maintain an infection prevention and control program for 2 residents by not properly storing denture and oxygen equipment when not in use. Resident #1 was a cognitively intact male with multiple chronic conditions, including diabetes mellitus type II, hypertension, coronary artery disease, chronic kidney disease stage III, generalized weakness, dehydration, hypothyroidism, gout, osteopenia, and BPH. He required staff assistance with ADLs, oral hygiene, and denture management, and his care plan included a mechanical ground diet with thin liquids and assistance with eating-related tasks. During an observation in the resident’s room, his dentures were seen inside a clear plastic cup on the side table without a lid, and another denture cup containing liquid was observed uncovered on the dresser. The Speech Therapist stated dentures should not be left uncovered because of contamination and potential infection if used without proper cleaning, and also noted the risk of the dentures falling and breaking. CNA A, CNA B, RN C, the DON, and the Administrator each stated dentures should be cleaned and stored in a designated denture cup or container, and that leaving them exposed created a risk for contamination, infection, damage, or loss. Resident #39 was a female with diagnoses including hypothyroidism, hypertension, allergic rhinitis, and insomnia, and her history and physical indicated she required supplemental oxygen via nasal cannula as needed to maintain oxygen at 90%. Her MDS showed moderately impaired cognitive skills, and her care plan identified potential altered respiratory status and difficulty breathing, with staff interventions to monitor respiratory changes and pulse oximetry. During observation, her nasal cannula was hanging behind the bed’s headboard and touching the floor while the concentrator was on, and the resident stated she did not remember when she had last worn it and needed assistance. CNA A, the DON, CNA B, RN C, and the Administrator stated the cannula should be cleaned, bagged, labeled, and stored when not in use, and that leaving it on the floor or exposed was not acceptable.

Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.
