Failure to Provide Needed Nail Care and Personal Hygiene Assistance
Summary
The facility failed to provide necessary assistance with ADLs to maintain good grooming and personal hygiene for multiple residents whose care plans and assessments identified a need for staff help with personal hygiene and nail care. The deficiency involved Resident #13, Resident #15, Resident #54, Resident #65, Resident #128, Resident #129, and Resident #111, with observations showing fingernails or toenails that were long, untrimmed, or unclean despite documented needs for staff assistance. Resident #13 had severe cognitive impairment, required partial/moderate assistance with personal hygiene, and had a care plan directing staff to assist with nail care as scheduled or as needed. During observation, the resident stated her nails were long and not her preference, and her fingernails appeared jagged with black residue underneath. She said she had received a shower that day but was not offered nail trimming services during or after the shower. Resident #15 had severe cognitive impairment, required setup or clean-up assistance for personal hygiene, and had a care plan calling for staff assistance with personal hygiene needs. During observation, he stated he had been at the facility for about two months and had never received fingernail trimming. His nails were observed extending beyond the finger with dark residue underneath, and he said he had received showers but was not offered nail-cutting services. Resident #65 required substantial/maximal assistance for personal hygiene and had a care plan for staff assistance with personal hygiene needs; during observation, he had long dirty fingernails and stated he wanted staff to cut them when they got long, but he usually had to request the service because he was diabetic. Resident #54 required maximal/substantial assistance with personal hygiene and had a care plan directing staff to check nail length and trim and clean nails on bath day and as necessary. Resident #111 required extensive staff assistance with ADLs, had impaired cognition and mobility, and his care plan included staff assistance with personal hygiene and ADLs. During observation, his toenails were long with yellowish discoloration, and he stated he did not like them long, did not have nail clippers, and had requested help from CNAs but had not been helped. Resident #128 and Resident #129 both had severe cognitive impairment, were dependent for personal hygiene, and had care plans directing staff to check nail length and trim and clean nails on bath day and as necessary; during observation, their fingernails were about a quarter inch from the nail bed and clean, and they were unable to answer questions.
