Summary

The facility failed to provide pharmaceutical services to meet the needs of a resident by not ensuring the physician was notified when blood sugar readings were over 300, as required by the resident’s insulin sliding scale order. The resident was admitted with diagnoses including abnormal glucose, nontraumatic intracranial hemorrhage, malignant neoplasm of the bladder, and dysphagia. His quarterly MDS reflected a BIMS score of 13, indicating intact cognition, and his care plan identified diabetes-related risk with interventions to administer medications as ordered and promptly report abnormal lab results and significant clinical findings to the physician. The resident’s physician orders included Insulin Aspart before meals with sliding scale parameters and a directive to notify the MD if blood sugar was less than 60 or greater than 300. The June 2026 MAR showed blood sugar checks and insulin administration multiple times daily, and readings over 300 occurred on several occasions, including 305, 400, 386, and 389. During interview, the LVN who was the resident’s charge nurse stated she initially believed the notification threshold was over 400, then acknowledged after reviewing the order that the physician should have been notified when readings were over 300. She said the resident’s blood sugars were high throughout the day and that she had not notified the NP for those readings. The DON stated it was the responsibility of nursing staff and medication aides to follow medication orders and acknowledged that the LVN should have contacted the NP when blood sugar readings were over 300. The DON also stated she failed to recognize that the notification had not occurred. The NP confirmed he had been caring for the resident for two years, that the resident’s blood sugars often ran high, and that the resident was still undergoing titration of insulin. The facility’s medication administration policy required nurses and medication aides to read and follow precautionary or instruction labels and verify medication orders for accuracy.
