Summary

The facility failed to ensure adequate supervision and assistance devices were used to prevent accidents during transfers for two residents with severe cognitive impairment and significant assistance needs. Resident #2 had a BIMS score of 0, was always incontinent, and required maximum assistance with transfers, toileting, and personal hygiene. Her care plan called for extensive assistance of one staff for transfers. Resident #3 had a BIMS score of 3, was always incontinent, required substantial to maximum assistance with transfers, toileting, and personal hygiene, and had sustained two falls without injury since the prior assessment. Her care plan called for two staff for transfers. On 06/27/26, Resident #2 was observed being transferred from her wheelchair to bed by CNA A and CNA B. Both staff positioned the wheelchair next to the bed and lifted the resident onto the bed with one arm under her armpit and the other hand grabbing the back of her pants. The resident was not bearing her full weight on her legs, and no gait belt was used even though a gait belt was observed hanging on the door in the resident's room. On the same day, Resident #3 was transferred from her wheelchair to bed by CNA A and CNA B. Both staff lifted the resident from the chair onto the bed with their arms under her armpits, and no gait belt was used during the transfer. During interview, CNA A and CNA B stated they were not supposed to lift residents under their arms because it could cause injury to the residents' shoulders, and they stated they were supposed to use a gait belt but did not see one in the room. The Director of Rehab stated it was not acceptable to lift someone under their armpits because of the potential for injury to the resident and caregiver, and the DON stated staff should use a gait belt when transferring a resident. The facility policy stated residents are to be handled and transferred safely, handling aids may include gait belts, and transfers are to be performed according to the resident's individual plan of care.
