Inaccurate MDS Assessments for Pressure Injury, Dental Condition, and Penile Slit
Summary
The facility failed to ensure resident assessments accurately reflected resident status for 3 of 24 residents reviewed. For Resident #3, the latest quarterly MDS did not document a stage 4 pressure ulcer on the right hip, even though the care plan identified a stage 4 pressure injury and wound records showed ongoing treatment and measurements for that wound. The quarterly MDS skin section instead indicated no unhealed pressure ulcers or injuries and no unstaged pressure ulcers, despite physician and skin assessment documentation showing the stage 4 wound was present during the assessment period. Resident #35’s MDS assessments also did not accurately reflect her oral/dental condition. Her quarterly MDS did not complete Section L for oral/dental status, and her annual MDS indicated none of the listed oral conditions were present. This was inconsistent with prior dental documentation showing severe calculus, plaque buildup, poor oral hygiene, poor gum health, gross decay, bone loss, and multiple non-restorable loose teeth with recommendations for extraction of numerous teeth. During observation, her teeth were brownish, some were broken and chipped, and a strong oral odor was noted. She reported oral pain, said she had not been to the dentist in a long time, and stated she wanted to go. Resident #51’s significant change MDS did not reflect a penile slit that was observed during catheter care. The assessment documented an indwelling catheter and toileting dependence but did not include any skin concerns related to the penile slit. The care plan addressed catheter insertion, changes, maintenance, and monitoring for skin breakdown, but did not address the penile slit or the use of straps to secure the catheter. During observation, staff found no leg strap or Statlock securing device in place, and the penile slit was measured by the treatment nurse. The MDS Coordinator stated the omission from the MDS and care plan was an oversight, and the DON stated the expectation was that MDS assessments and care plans would be accurate to ensure continuity of resident care.
