Infection Control Lapses During Wound Care, Incontinence Care, and Medication Handling

Summary

The facility failed to maintain an infection prevention and control program for four residents reviewed for infection control. Resident #5 had diagnoses including type 2 diabetes mellitus, vascular dementia, chronic kidney disease, hyperlipidemia, cellulitis, and major depressive disorder, and was always incontinent of bowel and bladder. The resident had a full thickness non-pressure wound to the right lower extremity and a physician order for daily wound treatment. During observation, there was no enhanced barrier precaution sign and no PPE seen by the resident’s room. An LVN stated the resident had a leg wound and should have been on enhanced barrier precaution, but was not at the time because the wound had reopened a couple of days earlier. Resident #10 had diagnoses including chronic kidney disease, dysphagia, colostomy status, type 2 diabetes mellitus, hyperlipidemia, and a pressure ulcer of the right buttocks. During wound care, an LVN cleaned the wound and did not change gloves or wash hands before applying treatment and a clean dressing. The LVN stated she forgot to change her gloves after cleaning the wound, but should have changed them and sanitized her hands. The DON stated staff should change gloves after cleaning a wound to prevent cross contamination and infection. Resident #63 had diagnoses including Parkinson’s disease, cardiomegaly, dysphagia, hypercholesterolemia, dementia, and anxiety disorder, and was coded with an indwelling catheter and bowel incontinence. During catheter care, a CNA changed gloves after cleaning the resident and before applying cream and touching clean briefs, but did not sanitize her hands before putting on new gloves. The CNA stated she did not realize hand sanitizer was required between glove changes. Resident #92 had diagnoses including a sacral fracture, malignant neoplasm of the sigmoid colon, type 2 diabetes mellitus, and hypertension, and had a colostomy. During medication preparation, an LVN placed a pill in the palm of her bare hand before putting it into the medication cup. The LVN stated she knew she should not have touched the pill with her hand because of the risk of contaminating it before giving it to the resident. In each instance, the DON stated staff should follow infection control practices to prevent contamination and infection.
