Summary

The facility failed to appropriately discharge one resident and did not ensure he was not discharged home without Home Health services to assist with ongoing care needs. The resident was admitted with hepatic encephalopathy, hypertensive heart disease with heart failure, alcoholic polyneuropathy, and alcoholic cirrhosis of the liver without ascites. His MDS reflected a BIMS score of 10, indicating moderate cognitive impairment, and the care plan identified wandering/elopement risk and impaired cognitive function, with interventions including 1:1 supervision and monitoring for changes in cognition. The care plan did not include discharge planning information. During the stay, multiple staff described the resident as confused, exit-seeking, and needing supervision. Staff reported he repeatedly said he wanted to go home, packed his belongings, attempted to leave, and was placed on 1:1 monitoring because of elopement concerns. The DOR stated he needed supervision due to cognitive impairments and confusion, and the Speech Therapist stated his cognitive status and problem-solving were low with limited recall. The resident’s external doctor wrote that he should not live unsupervised or be allowed to drive because of cognitive impairment, and the family member told the facility that discharge home would be unsafe because he lived alone. The discharge process occurred after the resident continued requesting to leave, and the facility contacted APS to assess the home environment. The SW stated she made a referral for Home Health services, but Admissions later stated the referral was not received until much later. The resident was discharged by Uber to home, and the family member reported she was not notified of the discharge and did not know how he got home. After discharge, the family member stated she had to organize his medications in a pill box and observed that he took extra doses because he did not know how to take them. During the post-discharge interview, the resident could not clearly identify the facility, asked what day it was, and stated he did not know which medications to take or when to take them.
