During a confidential group interview on 06/10/26 at 10:30 AM, 7 residents stated they were not aware of their right to access the facility’s survey results and said they would like to review them but did not know where to find them. On 06/11/26 at 9:38 AM, the Activity Director stated she had worked at the facility for over a year and had not informed the resident council of the right to review survey results or where they were located. She said the survey results binder was on a table near the front entrance. On 06/11/26 at 9:51 AM, observation showed the binder behind pamphlets at the front entrance on the table to the right, with no visible signage indicating where the survey results were located. The Administrator stated on 06/11/26 at 9:58 AM that residents and families were informed at admission and that he had spoken to resident council in the past about the right to review survey results and where they were located. Review of the facility’s undated Resident Rights Guidelines policy showed general resident rights guidance but did not specify the right to review survey results without having to ask.

Resident #2’s responsible party also stated the information was not posted in a location that made it readily accessible. S3 Activities Director and S4 Social Services Manager both indicated the notice was not posted as required, and S1 Administrator confirmed there were no notices posted and stated she was unaware the information was required to be posted.
