Missing COVID-19 Immunization Documentation
Summary
The facility failed to develop and implement policy and procedures to ensure residents were offered, received, or refused the COVID-19 immunization, and failed to maintain documentation of vaccination status for 5 of 6 residents reviewed for immunization compliance. The residents identified were Resident #2, Resident #3, Resident #11, Resident #60, and Resident #84. The facility also failed to develop a COVID-19 immunization policy that addressed documentation in the electronic medical record for residents who had received the vaccine or who had not received it due to medical contraindication or refusal. Resident #2 was a male with diagnoses including unspecified cerebral infarction, type 2 diabetes mellitus, and end stage renal disease, and his quarterly MDS showed a BIMS score of 15 with COVID-19 vaccination not up to date. Resident #3 was a female with diagnoses including hypokalemia, muscle weakness, and essential hypertension, and her quarterly MDS showed a BIMS score of 10 with COVID-19 vaccination not up to date. Resident #11 was a male with diagnoses including cerebral atherosclerosis, type 2 diabetes mellitus, and hemiplegia, and his quarterly MDS showed a BIMS score of 13 with COVID-19 vaccination not up to date. Resident #60 was a female with diagnoses including nontraumatic intracerebral hemorrhage, anxiety disorder, and COPD, and her quarterly MDS showed a BIMS score of 15 with COVID-19 vaccination not up to date. Resident #84 was a female with diagnoses including senile degeneration of brain, type 2 diabetes mellitus, and essential hypertension, and her quarterly MDS showed a BIMS score of 0 with COVID-19 vaccination not up to date. Record review showed no documented evidence of COVID-19 immunization status for Resident #2 and Resident #60, and no documented evidence of refusal for Resident #3 and Resident #11. For Resident #84, the record showed COVID-19 vaccination was not up to date, but the documentation section was repeated in the record review. During interview, the DON stated the ADON was responsible for coordinating and overseeing the immunization program and that refusal documentation was expected to be in the ADON's office, but the records could not be located. The DON stated the refusal documentation was an area needing attention and would be uploaded and updated in PCC. The ADON stated residents were offered COVID-19 vaccination on admission, refusals were entered into PCC, and signed refusal forms and VIS were kept in binders in her office, but she could not locate the records for the five residents and did not explain how immunization compliance was monitored.
