Baseline care plan not completed after resident re-admission
Summary
The facility failed to develop and implement a baseline care plan within 48 hours of Resident #44’s re-admission. Resident #44 was a [AGE]-year-old female with chronic respiratory failure, muscle wasting and atrophy, hypothyroidism, anemia, COPD, lack of coordination, arthritis, nicotine dependence, systemic inflammatory response syndrome, and anxiety disorder. Her record showed she had been admitted previously, then discharged against medical advice to home on 04/30/26, and later returned to the facility on 05/02/26. The survey found that the baseline care plan for this re-admission was not completed within the required timeframe. Record review showed Resident #44’s MDS reflected the 05/02/26 entry as a reentry from the original admission. Her quarterly MDS showed a BIMS score of 15 and that she used a manual wheelchair, needed setup or clean-up assistance for meals and oral hygiene, and required partial/moderate assistance with other ADLs. Her discharge MDS and discharge summary documented that she had left the facility against medical advice, was discharged to home in a family vehicle, and had no home health care services arranged. The record also noted that Resident #44 and her RP did not want to sign the discharge summary. The facility’s care plan report showed focus areas, goals, and interventions that had been initiated on 04/01/26 and revised and cancelled on 05/04/26. During interviews, the MDS Coordinator stated she was responsible for completing care plans but did not realize she had to do another care plan when Resident #44 returned after leaving AMA. The DON stated the facility had 24 hours to complete a baseline care plan and acknowledged that if there was not a care plan, the resident could be at risk of not receiving proper care. The ADM stated the initial care plan should be completed within 48 hours and said Resident #44 did not have a care plan because her profile had been deleted when she left the facility. He also stated he had not been aware that a resident returning after leaving AMA required a whole admission process that included the initial care plan.
