Failure to Provide Nail Care and Hygiene Assistance
Summary
The facility failed to ensure that residents who were unable to independently complete activities of daily living received the necessary assistance with grooming and personal hygiene. During observation and record review, three residents were found with long fingernails that had visible dirt or brown substance underneath them. The deficiency involved Resident #8, Resident #66, and Resident #70, all of whom had care plans and MDS assessments indicating they required assistance or supervision with personal hygiene. Resident #8 was a female with diagnoses including dysphagia, muscle wasting, lack of coordination, unsteadiness on feet, pain, vomiting, hypertension, and localized edema. Her quarterly MDS reflected a BIMS score of 15 and need for supervision or touching assistance with personal hygiene. Her care plan identified an ADL self-care performance deficit and directed staff to encourage participation in self-care. On observation, she was lying in bed with long fingernails and dirt underneath them. She stated she had asked staff to cut her nails, did not know when they were last cut, and said having them cut would make her feel much better. Resident #66 was a male with diagnoses including Alzheimer's disease, cellulitis, cerebral infarction, lack of coordination, and difficulty walking. His quarterly MDS reflected a BIMS score of 15 and need for supervision or touching assistance with personal hygiene. His care plan included podiatry referral as needed for foot and nail care, and records showed he last saw the podiatrist on 03/03/2026, when his nails were cut and filed. On observation, he was sitting in his wheelchair with long fingernails and a brown substance underneath them. He stated he wanted his fingernails cut and cleaned and did not know when staff last cut them. Resident #70 was a female with diagnoses including muscle wasting, muscle weakness, left shoulder pain, embolism and thrombosis, peripheral vascular disease, hypertensive heart disease with heart failure, and heart failure. Her quarterly MDS reflected a BIMS score of 05 and dependence for personal hygiene. Her care plan included nail care referral to podiatry as needed, and records showed her last podiatry visit was also on 03/03/2026. On observation, she was lying in bed with long fingernails and a brown substance underneath them. She stated she had been asking staff to cut her fingernails, that no one had tried to cut them, and that she wanted them cut and cleaned.
